Registered Nurses and Certified Social Workers work with patients and their families for post acute discharge planning needs, crisis counseling and community outreach. Community referrals and resources as well as placement into a skilled nursing facility, rehab, assisted living, or a sub-acute facility are provided. Home care services are arranged for patients who qualify including, nursing care, home medical equipment and respiratory care needs. Patient Review Instruments (PRI's) are completed for some patients in the hospital to determine the level of care appropriate for them and to help plan their needs upon discharge. Office hours are Monday-Friday 8:00am to 5:00pm and Saturday & Sunday 8:00am-4:00pm.
