St. Catherine of Siena Medical Center recognizes that the care and well-being of patients does not stop at the walls of the hospital. A comprehensive array of services, complemented by the full resources of the entire Catholic Health Services organization for transition planning is offered to patients and the community.

The dedicated registered nurses and certified social workers work with patients and their families for post acute discharge planning needs and crisis counseling. Staff coordinates community referrals and resources as well as placement into skilled nursing, acute and sub-acute rehab, and assisted living facilities.

Home care services are arranged for patients who qualify, including nursing care, physical therapy, home medical equipment and respiratory care needs. Patient Review Instruments (PRI's) are completed for some patients in the hospital to determine the level of care appropriate for them and to help plan their needs upon discharge.
